Keep in Mind

A referral letter from a doctor (modern medicine) is required at the time of registration.

The child’s name, age, parent’s name, phone number, exact address, income and other details are required at the time of registration.

After consultation, the parents are supposed to fix the follow up date and time.

Please take in hand the necessary food and clothes for the child. For small children, please take diapers along with you while you come to CDC.

Please stick to the given time when coming to register the child.

The treatment details of the child from birth must be brought with the parent.

The clinics at CDC does not function on Second Saturdays, Sundays and on pre-determined public holidays.
